[civsa] Protocol for checking out a wheelchair to campus visitor

Hi Liz,

We have three wheelchairs that are available for guest use. If a guest
calls ahead of time and asks to reserve one we will put one to the side for
that guest. We also ask for a state ID just like UC-Boulder.
