Trent,

Here are 2 examples of the document I use at Augustana.  We call it an
Expectations Agreement.  I amend the document to fit each student's issues
using citations from their Employment Agreement. We meet with the student
to discuss this document and our plan for follow-up.  Luckily, I haven't
had to use this document in over a year.
