[civsa] Exit Surveys: Post Tour

Hello Everyone,
I am reaching out to seek help on our exit surveys.
After every tour we have a 7 question survey in which they rank whether these categories: exceeded expectations, met expectations, or were below expectations.
This is then followed up by written responses about their tour guide (what can they improve on, what did they do well on) and their overall campus experience.
We are curious whether these questions are effective in receiving critical campus feedback to improve our tours in the future.
We are currently distributing them via paper surveys, but are switching to a QR scan and online form they can fill out- we are trying to go as green as possible.
With that being said my questions for you all are:
What type of questions do you ask on your surveys?
How do you distribute your surveys to your guests?
