[civsa] Tours' Surveys

Here are the questions (attached) that we currently use on our Group Tour evaluation survey. Our survey was created using the survey tool Qualtrics and is distributed by email following the completion of the tour. The submissions are then collected in Qualtrics where we can review them.

UCLA Campus Tours is in the process of developing a survey to send out to guests after individual and group tours. Do any of you have any surveys you wouldn't mind sharing? Also, logistically, how does your survey get distributed (email, paper following tour, etc) and collected?
