[civsa] Waivers

Hi Catie,

Greetings from NC State!  We have a variety of pre-college programs that
require waivers.  We typically e-mail a link to the forms, and the
participants and their parents can sign them and scan them in and send them
back, or take a picture and send them back, or they can mail them in or
they can bring them to the event in person.  We do not use electronic
signatures.
