[civsa] Request for Performance Appraisal feedback

Good Afternoon,

I would really like to know about your college's performance appraisal
process.   If you have anything different to add that would be helpful to
share.   I am on a task force this week and would appreciate others
feedback on their process.

Effectiveness, smooth transition, metrics used to evaluate.

1.  Employees receive a self performance appraisal worksheet a month prior
to the review to submit to supervisor. (4 questions)
2.  Supervisor sends their write up to HR, when this is returned then
Performance Appraisal with employee (6 questions)
3. Appraisal is signed and sent to HR

I would appreciate any feedback as soon as you could, and thank you for
your time.
