Hello Laura and CIVSA!

We host ours in Qualtrics (which is like Survey Monkey), but here's a Word Document with the questions we ask on our survey. Every student that visited gets this form sent in their "Thank you for Attending" email one day after their visit.

Greetings!  We are revamping our campus tour evaluations and I'd love to get ideas for questions that everyone asks!  If you don't mind sharing your evaluations, could you please send them to me?  I'd love to see what everyone else is doing.

Also, if you have different evaluations for group tours or special tours, that would be amazing, too!
