Hello!
We do offer one domestic fly-in to each out-of-state admitted student. (We will pay for the domestic leg of an international student's trip.) We also pay for one parent or guardian to accompany them. We provide the flight, but do not provide hotel accommodations or ground travel. Students typically use this either for our Scholarship Weekend (exclusive invitation) or for one of our Admitted Students Days (open to all admits), but we allow them to use it at other times if they cannot attend those events.
We're in the Slate world, so we add a fly-in request form to the relevant students' Slate portal and set rules on the form that prevent submission from unqualified students. We typically have a separate flight request form for each event. We have a designated counselor that determines whether the student has already received a flight, purchases all of the flights, and sends confirmation e-mails to the student. The request form is pretty extensive and collects all the information needed to book the flights (full names as shown on ID, gender, birthdate, addresses, e-mails, phone numbers; 1st and 2nd choice of airports, departure date, landing times, return dates).
We do also have a policy that any flight change fees are the responsibility of the student and that students who miss the event are liable for the cost of the tickets. This is stated on the request form, and the student must acknowledge those terms. This is not strictly enforced in the event of emergencies, but we have it in place just in case.
