[civsa] Group visit contract

Hey everyone,

Just got a call from our Legal Department and they want us to set up a
Group Visit contract to spell out, and set the expectations for what the
responsibilities are for the visiting group (everything) and for us
(nothing).  I am all on board for this and would love if anyone, who
currently has groups sign a contract or code of conduct notice prior to
their visit, could send me a copy so I can see how to script ours.
