[civsa] Post-Tour Evaluations?

We send out an email after each tour to those that were in attendance. The link we provide is below.

However, for our full-day programs we do a paper evaluation which they complete before they leave the closing ceremony.

I was just curious if anyone sends out evaluations for their guests to complete after a tour? If so, how do you provide those (email, hard copy), how soon after the tour, is there an incentive for them etc. and do you mind sharing the questions you ask on the evaluation?
