[civsa] student evaluations

Hi Everyone and Happy Friday!
I would like to give my student ambassadors a quarterly evaluation.  The
person who had this job prior to me never did evaluations with them so I'm
not sure where to start.  The staff evaluations I feel are too detailed I'm
looking for something more simple.  Can anyone please email me the
evaluation form you use for your students?
